Insurance Coverage

Weiser Skin MD may not be a participating provider with your health insurance plans. Depending upon the exact terms and conditions of a patient’s coverage, medical visits are generally eligible for submission to, and may be covered by, most health insurance plans. However, coverage and benefits vary. We strongly encourage all patients to verify their insurance benefits and eligibility directly with their insurance provider prior to their appointment.

Cosmetic Consultations

Please note that consultations involving cosmetic or non-medical concerns—including, but not limited to, aesthetic evaluations, elective procedures, and product recommendations—are not covered by insurance and will incur an out-of-pocket consultation fee. Fees are as follows:

• $350 or $450, depending on the provider selected. Cosmetic consultations may include topics such as facial aesthetics, body contouring, skin rejuvenation, and personalized skincare regimens. Same-day treatment options may be available upon request and provider availability.

Cancellation Policy

To avoid cancellation fees, we require at least two (2) full business days’ notice for any rescheduled or canceled appointment. Late cancellations or missed appointments will result in the following charges:

• Cosmetic Appointments: $350 or $450 per 30-minute scheduled time block, based on provider
• Medical Appointments: $100 flat fee

Credit Card Surcharge

Effective September 1st, 2025, a surcharge will be applied to all credit card transactions. This fee does not exceed our cost of acceptance.

By requesting your medical records via email or text, or by communicating with our office through these channels, you acknowledge and accept that these forms of communication are not HIPAA-secure for transmitting Protected Health Information (PHI) and consent to the use thereof. Any information shared via email or text is transmitted at your own risk.

Weiser Skin MD is not liable for any potential unauthorized access, data breaches, or disclosure of PHI resulting from the patient consented to use of unsecured communication methods, including but not limited to email and SMS/text messaging.
